In 2024, Cognis advised the Nigerian Revenue Service on the TaxPro Max → Rev360 cutover. Two weeks before go-live, the reconciliation team found that 47,000 taxpayer records had collided in the destination — duplicate entities under slightly different names. The same bug class destroyed TSB Bank UK (2018), SAAQ Quebec (2025, $1.1B overrun), and Revlon's SAP rollout. Migratio's Reconciliation Agent and Naming Library are the answer.
